Appointment/Procedure Preparation

To schedule an appointment, please call our office at (937) 208-7776, from 8:00 a.m. to 5:00 p.m.

What to Expect

New patients must complete a Patient Registration form prior to seeing the doctor. We will mail this form to you when you schedule your appointment. You may also download the form here.

New patients should arrive 10-to 15 minutes early so we can make copies of your health insurance information.

Please bring your medication bottles or a list of the medications you are currently taking. Fasting is unnecessary, unless requested by your physician.

If you have a co-pay, please be prepared to make payment at the time of service. 

After Your Visit or Procedure

If you do not have a return appointment, test results are released to a patient's MyChart account automatically 3 days after the test. Please contact us if you have not heard from us within two weeks of your test.

Please request prescription refills at your scheduled office visits. If you do need a refill before a scheduled visit, please call the office or send a MyChart message. Please allow 48 to 72 hours for the refill, since a physician must approve all prescription refills.
